A death certificate is a legal certificate issued by the Government to record a person’s death, stating the date, fact and cause of death. According to the Registration of Birth & Death Act, 1969, every death has to be registered with the concerned State Government and death certificate will be issued to nearest relatives of the deceased. The Municipal Register/CMO acts as the Death Registrar and is authorized to issue the death certificate. In this article, we look at the procedure for obtaining the Madhya Pradesh death certificate in detail.

Registration of Birth & Death Act, 1969

Registration of Birth & Deaths Act, 1969 was enacted by the Parliament, to regulate the registration of births and deaths in India. As per the provision of this Registration of Birth & Death Act, every death must be registered within 21 days of the occurrence with the concerned State Government.

Registering Death in Madhya Pradesh

The following persons are responsible for registering the death in Madhya Pradesh:

  • Death occurs in the house: The head of the household is eligible to register the death in the concerned Registrar Office.
  • Death occurs in hospital: A person who is authorized by the medical institution is responsible for registering the death in the respective Registrar Office.
  • Death occurs in jail: The jail in-charge can register the death with the concerned Registrar Office.
  • Death occurs in public place: The local police in-charge or headman of the village can register the death.

Documents Required

To register the death in Madhya Pradesh, submit the following documents:

  • Proof of birth of the deceased.
  • The affidavit which contains the time and place of death.
  • Application Form
  • Passport size photograph of the deceased.

Fee Details

The government fee to obtain the death certificate in Madhya Pradesh is provided below:

Procedure for Obtaining Death Certificate

To register for a death certificate under Madhya Pradesh Government, kindly follow the steps given below:

Step 1: The applicant must go to the official website of Madhya Pradesh Government.

Step 2: Click on the “Death Registration” link on the home page.

Step 3: Select your city from the dropdown list and then click on the “Continue” button.

The application for the death registration form will be available, as shown below:

Step 4: Fill all the necessary details in the application form and click on the “Upload” button.

Step 5: After the submission of the form, the system would calculate and show the required fees to be paid for the registration.

Step 6: Once the payment has been successful, you will receive a registration number on the payment receipt as the acknowledgement.

Track Application Status Online

The applicant can view the death certificate application status online by using the official portal of Madhya Pradesh. Click on the Citizen Service Application Status and enter your application number to view the status of your applications.

Download Death Certificate Online

The user has to login to the official portal and clicks on the “Download Death Certificate” option under the citizen services on the home page. Enter the application number and click on the “Get Certificate” button to download the death registration certificate online.
