The Chief Registrar of birth is responsible for issuing the birth certificate under the State Government of Madhya Pradesh. Purpose of Birth Certificate

Every resident of Madhya Pradesh must register the birth and apply for the birth certificate, as it proves the nationality of a person. The birth certificate is also useful for various purposes like admission in educational institutions, obtaining a passport, voter card, marriage certificate, driving license, etc.

Registering Birth

The registration of birth has to be done within 21 days from the date of the birth by the following specified person.

If a birth occurs in a hospital, the person authorized by the medical officer is required to record the birthIf a birth occurs in a house, the household head or adult person in the house is eligible to record the birth with the concerned authority. Finally, if birth occurs in a public place, the head of the corresponding village in the case of a town or the officer in charge of the local police station is required to register the birth.

Documents Required

The following specified documents are to be provided at the time of applying for Madhya Pradesh birth certificate:

  • Proof regarding the place of birth (where the child was born) from the hospital.
  • Parent’s identity proof.
  • Marriage certificate of the parent, optional.

Online Application Procedure

To register for a birth certificate under Madhya Pradesh Government, follow the steps given below:

Step 1: Please visit the official website of Madhya Pradesh Government.

Step 2: Click on “Birth Registration” option which is visible on the homepage of the portal.

Step 3: Select your city from the drop-down list and click on the “Continue” button.

Step 4: After selecting your respective city on the next page, the application form will open.

Step 5: Fill up the application with the required details and upload the scanned documents.

Step 6: Finally click on the “Continue” button for successful registration.

Note: After submitting the application, the applicant will receive a registration number as an acknowledgement to check your application status.

Track Application Status Online

Birth certificate application status can be verified online by using the official portal of Madhya Pradesh. You have to click on the Citizen Service Application Status and enter your application number to view the status of your application.

Download Birth Certificate

The user has to login to the official portal, and one needs to select the “Download Birth Certificate” option under the citizen services on the home page. Now you need to enter the application number and click on the “Get Certificate” button to download the birth certificate online.
