Eligibility Criteria to obtain UP Caste Certificate
To apply for Uttar Pradesh caste certificate or Jathi Praman Patra, the following eligibility criteria should be met.
- Only residents of Uttar Pradesh can apply this certificate.
- Any person belonging to the caste or class listed in Schedule-1 of the Uttar Pradesh Public Services can apply for caste certificate.
Documents Required
Following documents need to be furnished for obtaining Uttar Pradesh Jati Praman Patra.
- Application Form in the prescribed format
- Caste certificate of father
- Self Declaration
- Address Proof – Ration card/Food Security Card, Voter ID, telephone bill or electricity bill can be produced
- Income certificate
- Certificates in respect to caste / warden / village head
- Passport Size Photograph
Applicable Fee
There is no charge for applying for Jati Praman Patra in the Tehsil office. However, you need to affix the court fee stamp of Rs.1.5 with the application form. If applying through an e-District local service centre, pay a service charge of Rs.20 for caste certificate processing.

Jati Praman Patra validity in UP is for a lifetime unless the reservation itself is scrapped.
Apply Caste Certificate UP through CSC centre
Procedure for applying UP caste certificate is explained in steps here.
Step 1: To apply for Uttar Pradesh Jati Praman Patra, Fill the application form duly according to your caste category. The application form for caste certificate is below for the applicant reference.

Step 5: CSC person will forward the request for caste certificate online to the concerned department. Note the application number for future reference.
Step 6: Process of the application process will be updated through mobile number.
After processing the application form, caste certificate will be issued. On receiving application accepted message, revisit the centre and give application number for downloading Jati Praman Patra.
Apply Caste Certificate through UP Online
For applying Uttar Pradesh Jeevan Praman Patra follow the step by step guidelines given here:
Step 1: Go to the official website of Government of Uttar Pradesh – UP Online.
User Registration
Step 2: In case of the new user to UP Online page, register in the portal to avail certificate services. Click on Register button.
Step 3: Enter the login ID and click on the check availability button, if the login ID entered is unique go for further process.
Step 4: Provide the following details for user registration such as Name, Date of Birth, Gender, Residential Address, District, Mobile number and Mail ID.
Step 5: Enter the security code shown here and click on Secure this option to register in the portal.
Step 6: Now, receive an OTP to the registered mobile number, enter the same for generating a password.
Login to Portal
Step 7: Once registration has completed, log in to the webpage using user name and password.
Note: Choose Citizen Option from the drop-down menu for login as a citizen.
Step 8: After login to the portal, select fill an application option.
Provide Details
Step 9: From service option, select Jati Praman Patra from the drop-down menu.
Step 10: Provide all mandatory details for caste certificate.
- Name
- Gender
- Parents’ name
- Address
- Age and Date of Birth
- Community, Religion
- The religion of father and mother also needs to be provided
Upload Documents
Step 11: Upload photo, self-declaration (furnished above) and Ration card.
Step 12: Once all details are provided, click on the submit button. Application number will be generated, keep it safe for future reference.
Status of Application
The applicant can verify the application status by visiting Uttar Pradesh e-District portal. Select application status option from home page.
Provide your application number and click on the search button to view the status of the application.

Verify and Download Certificate
Use the below instructions to verify and download caste certificate UP.
Once the Jati Praman Patra application is approved by the concerned authority, verify the details of the certificate from e-District Uttar Pradesh.
Enter the application number and certificate ID and click on the search button. Verify the details of the issued certificate.
After verification, the applicant can download caste certificate UP by login to the UP online site.
