- Income Proof provided by the Panchayat.
- Death Certificate of the deceased husband.
- Below Poverty Line ration card.

Applying for the Widow Pension Scheme
To apply for the Widow Pension Scheme, follow these steps:
Step 1:Visit one of the following offices to obtain the Application Form (Form-P):
- In rural areas, visit the office of the Block Development Officer.
- If residing outside Kolkata Municipal Corporation areas, visit the Sub-Divisional Officer's office of the respective Panchayat Samity.
- For those residing within Kolkata Municipal Corporation, visit the Controller of West Bengal's office.
Fill in the application form and attach all required documents listed above.

Step 3:Submit the completed application form and necessary documents to the office from which you obtained Form P.
