A death certificate is an official document obtained after registering a death with the concerned State government authority. In India, all deaths must be registered within 21 days on the occurrence of the event as per the provision of State’s Birth and Death Registration Act, 1969. There are several reasons to get a death certificate which declares the cause, location and time of death. The death certificate also serves as a proof for legal purposes to transfer pension benefits, life insurance, medical benefits and other official commitments. In this article, we look at the registration, required documents and application procedure for obtaining Himachal Pradesh death certificate in detail.

Death Registration

The following persons are eligible to register the death in Himachal Pradesh:

House:  The head of the household is eligible to register the birth in the concerned Registrar Office.

Hospital: The person who is authorised by the medical institution is responsible for recording or registering the death in the respective Registrar Office.

Jail: The jail in-charge can register the death with the concerned Registrar Office.

Public: The local police in-charge or headman of the village can record the death.

Fee Structure

The applicable charges for registering the death certificate are below:

  • If registering death within 21 days from the occurrence of death is free of cost.
  • If registering death after 21 days from the occurrence of death will cost a fee of Rs. 2
  • If registering death after 30 days from the occurrence of death will cost a fee of Rs. 5
  • If registering death after a year from the occurrence of death will cost a fee of Rs. 10

Required Documents

To apply for a death certificate in Himachal Pradesh produce the following documents along with the application.

  • Proof of a Birth Certificate
  • Ration Card or Government ID proof
  • Passport size photo of deceased
  • An affidavit mentioning the place, time of death

Required Information

While filling up the application form requires the following information from the applicant:

  • Details of the deceased person such as name, nationality, religion, gender and occupation.
  • Name of Father/Husband of the person
  • Date of death
  • Address of the person
  • Place of Death
  • Post-mortem report if any crime or accident causes death.
  • The report from concerned authority is required if the death happens in hospital or jail or police station

Step 1: Visit the home page of e District Himachal Pradesh and click on “Download Forms” option.

Step 2: Then click on the “Certificate” option on the next page.

Step 3: On the next page, where all the State Government application forms are available. Click on “Death Report Form (Form – 2 )” option below.

Step 3: Then click on ” PDF ” or “Fillable PDF”.  Take a printout and fill up the application form correctly with the required information.

Step 4: Finally, submit the duly filled application to the Panchayat Secretary/Panchayat Sahayak of Gram Panchayat of Himachal Pradesh along with the required documents.
