Food Safety Standards Authority of India (FSMS) Declaration is a form under rule 2.5.1 of the Food Safety and Standards Act, 2006 required to be signed by the Proprietor or Partner or Director of the entity applying for FSMS Food Product Approval or FSMS Food Business License. Through the FSMS Declaration, the owner of a food business in India can nominate an employee of the business in charge of establishment, branch or the unit to be responsible and liable for food safety or any contravention of the FSMS Act and rules/regulations.

The FSMS Declaration is a mandatory attachment while applying for FSMS Food Product approval or FSMS Food Business License.

The FSMS Declaration can be printed on the letterhead of the business. The undertaking must be signed by one ore more of the Proprietor or Partner or Director of the business applying for FSMS food product approval or FSMS Food Business License.
